Activity Description

Vertigo, a prevalent symptom in primary care and emergency settings, manifests as a sensation of motion, typically rotational, due to vestibular dysfunction. To ensure accurate diagnosis, this condition requires differentiation from other forms of dizziness, such as presyncope, disequilibrium, or lightheadedness. Peripheral causes, like benign paroxysmal positional vertigo or vestibular neuritis, generally carry a favorable prognosis, while central causes, such as cerebrovascular disease, neoplasms, or demyelinating disorders, often have a poorer outlook if not promptly diagnosed. Symptoms may overlap, but specific clinical and neurological findings can reliably distinguish between peripheral and central etiologies. Misdiagnosis or delayed treatment can lead to complications, emphasizing the need for thorough history-taking and examination to guide management, which may include repositioning maneuvers, medications, or further investigation for central causes.

The course enhances clinician competence in evaluating and managing vertigo by focusing on key clinical features, diagnostic techniques, and evidence-based guidelines. Participants learn to collect detailed neurological histories and perform targeted examinations to differentiate peripheral from central vertigo causes, improving diagnostic accuracy. The curriculum covers treatment options, such as canalith repositioning for benign paroxysmal positional vertigo or medical management for vestibular neuritis, and emphasizes patient education on recognizing symptoms requiring further evaluation. Interprofessional collaboration, involving neurologists, otolaryngologists, radiologists, and physical therapists, is highlighted as critical for coordinating diagnostic workups, such as imaging or vestibular testing, and tailoring treatment plans. This teamwork enhances prompt diagnosis, reduces complications, and improves patient outcomes by ensuring comprehensive, multidisciplinary care.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Identify key clinical features of vertigo, including rotational motion sensations, to distinguish it from other forms of dizziness, such as presyncope or lightheadedness.

  • Differentiate peripheral causes (eg, benign paroxysmal positional vertigo, vestibular neuritis) from central causes (eg, cerebrovascular disease, neoplasms) through detailed neurological history and examination.

  • Interpret the key systemic and neurological examination findings that distinguish central vertigo from peripheral vertigo.

  • Collaborate and communicate with other interprofessional healthcare team members to improve clinical outcomes for patients affected by vertigo.
