Activity Description

Tracheostomy is an important airway adjunct involving the surgical creation of a stoma in the anterior trachea, typically between the 2nd and 3rd tracheal rings, to establish a stable airway. The procedure may be performed surgically or percutaneously, with placement of a tracheostomy tube allowing direct ventilation and reduction of airway resistance. Indications include acute upper airway obstruction, prolonged mechanical ventilation, severe obstructive sleep apnea, and facilitation of pulmonary hygiene in patients with neuromuscular weakness. Relative contraindications include uncontrolled coagulopathy, local infection, and anatomical abnormalities that impede safe access.

Complications may be immediate, such as bleeding, pneumothorax, or misplacement, or delayed, including tracheal stenosis, infection, and tracheoesophageal fistula. Compared with prolonged translaryngeal intubation, tracheostomy reduces ventilator-associated laryngeal injury, eases weaning, and improves patient comfort, though it is more invasive and carries procedure-specific risks. Knowledge of indications, technique, and complications is essential for interprofessional airway management.

This activity for healthcare professionals is designed to sharpen learners' skills in performing tracheostomy and identifying suitable candidates for the procedure. Participants will enhance their understanding of procedural steps, indications, contraindications, and potential complications. The role of the interprofessional team in monitoring, preventing, and managing complications is emphasized, highlighting collaborative strategies to optimize patient outcomes and ensure safe, coordinated care for patients who require or have undergone a tracheostomy.

Target Audience

This activity has been designed to meet the educational needs of physicians, physician associates, nurses, pharmacists, and nurse practitioners.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Select appropriate candidates for tracheostomy based on clinical indications, airway status, ventilatory needs, and risk-benefit assessment.

  • Implement tracheostomy proficiently by adhering to established procedural steps, employing best airway management practices, and following recommended monitoring protocols to detect possible adverse outcomes.

  • Apply evidence-based, personalized approaches to managing tracheostomy complications.

  • Collaborate with other healthcare professionals to optimize tracheostomy administration and develop a comprehensive airway management plan.
