Activity Description

Asthma constitutes a chronic inflammatory airway disease affecting millions worldwide, characterized by airway hyperresponsiveness, bronchoconstriction, mucus hypersecretion, and airway remodeling. Status asthmaticus comprises a severe, life-threatening exacerbation marked by persistent bronchospasm unresponsive to standard bronchodilators and corticosteroids. Common triggers include viral respiratory infections, allergen exposure, medication noncompliance, and environmental pollutants. Risk factors for status asthmaticus include poorly controlled asthma, previous severe exacerbations, and comorbidities such as obesity or chronic rhinosinusitis.

Clinically, patients with status asthmaticus present with severe dyspnea, wheezing, tachypnea, and accessory muscle use. Diagnosis relies on clinical assessment and measures of airflow obstruction, particularly peak expiratory flow rate and arterial blood gas analysis, which often show hypoxemia and hypercapnia. Management requires prompt administration of high-dose inhaled β-agonists, systemic corticosteroids, supplemental oxygen, and, if necessary, intravenous magnesium sulfate or mechanical ventilation, especially for refractory cases.

Complications include respiratory failure, pneumothorax, and cardiac arrhythmias. Prognosis depends on rapid recognition and aggressive treatment. Delayed intervention increases mortality risk. Interprofessional care and monitoring are essential to optimize outcomes in status asthmaticus.

This activity for healthcare professionals is designed to enhance learners' competence in evaluating and managing status asthmaticus. Participants will broaden their grasp of the condition's etiology, risk factors, pathophysiology, and clinical presentation, as well as evidence-based diagnostic and therapeutic strategies. Improved skills will equip clinicians to collaborate successfully within interprofessional teams caring for individuals with this condition.

This activity has been designed to meet the educational needs of physicians, physician associates, nurses, pharmacists, nurse practitioners, and social workers.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Differentiate status asthmaticus from less severe asthma exacerbation based on clinical and diagnostic features.

  • Apply evidence-based strategies for managing status asthmaticus and mitigating its complications.

  • Improve patient understanding of treatment goals, asthma triggers, and the role of adherence in preventing status asthmaticus and its associated morbidity.

  • Collaborate effectively with the interprofessional team in developing short- and long-term care strategies for patients with status asthmaticus.
