Activity Description

Hyperthyroidism in pregnancy represents a significant clinical challenge due to its impact on both maternal and fetal health. Although less common than other thyroid disorders, untreated or poorly controlled hyperthyroidism may contribute to adverse outcomes such as preeclampsia, preterm birth, intrauterine growth restriction, and fetal thyroid dysfunction. Graves’ disease remains the most common etiology, resulting from autoimmune stimulation of the thyroid gland and excessive thyroid hormone production. Clinical manifestations include tachycardia, weight loss, heat intolerance, and goiter, though symptoms may overlap with normal physiologic changes of pregnancy, complicating diagnosis. Accurate assessment requires careful integration of symptom recognition with laboratory evaluation of thyroid and thyroid-stimulating hormone levels. Management is further complicated by the need to balance maternal symptom control with fetal safety, given the potential teratogenic risks of antithyroid drugs. Early recognition, systematic evaluation, and monitoring are essential to minimize complications and promote healthy maternal and neonatal outcomes.

This course improves clinician competence in recognizing the clinical features of hyperthyroidism during pregnancy, selecting appropriate diagnostic tools, and implementing evidence-based treatment strategies. Participants develop the ability to tailor therapy to balance maternal control of disease with fetal protection while avoiding complications from overtreatment or undertreatment. Clinicians strengthen their skills in patient counseling, longitudinal monitoring, and interpretation of laboratory findings within the unique context of pregnancy. Interprofessional collaboration enhances care, as obstetric clinicians, endocrinologists, nurses, pharmacists, and neonatologists coordinate treatment, monitor for complications, and prepare for delivery and postnatal management. Effective teamwork supports consistent application of guidelines, improves adherence to therapy, and fosters patient education, ultimately reducing morbidity and ensuring better outcomes for both mother and child.

At the conclusion of this activity, the learner will be better able to:

  • Differentiate between normal physiological changes in thyroid function during pregnancy and pathological hyperthyroidism.

  • Apply evidence-based management strategies to balance maternal symptom control with fetal safety, incorporating current clinical guidelines for pharmacologic and nonpharmacologic interventions.

  • Implement evidence-based options for the management of hyperthyroidism in pregnant patients to ensure maternal and fetal safety.

  • Collaborate with an interprofessional healthcare team to optimize the management of hyperthyroidism in pregnancy and improve maternal and fetal outcomes.
