Activity Description

Dislocations of the finger joints are a common hand injury that can occur at the distal interphalangeal, proximal interphalangeal, or metacarpophalangeal joints. The direction of the dislocation may be dorsal, volar, or lateral, often involving damage to soft tissue structures such as the radial and ulnar collateral ligaments, volar plate, joint capsule, and the extensor mechanism. Prompt recognition and intervention are essential to reduce the joint and restore stability, as untreated or improperly managed dislocations can result in long-term complications, including deformity, joint stiffness, chronic swelling, functional loss, posttraumatic arthritis, and persistent pain. This course outlines the epidemiology, relevant anatomy, physical examination findings, imaging techniques, treatment protocols, surgical considerations, and possible complications associated with finger dislocations. Also emphasized are the clinician’s responsibilities to identify potential complications, understand the mechanisms of injury, and recognize when an orthopedic or hand surgery consultation is warranted.

The course equips participants with the knowledge and skills to accurately assess and manage finger dislocations, while identifying key factors that contribute to complications and poor outcomes. The course focuses on effective diagnostic evaluation, joint reduction techniques, and postreduction management strategies to promote optimal recovery outcomes. Collaboration with an interprofessional team—including orthopedic surgeons, emergency medicine clinicians, radiologists, nurses, physical therapists, and occupational therapists—enhances patient outcomes by ensuring comprehensive, coordinated care. This integrated approach supports timely diagnosis, appropriate intervention, and individualized rehabilitation plans, which are critical in preserving joint function and minimizing the risk of chronic disability.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Identify the anatomical structures commonly involved in finger dislocations, including collateral ligaments, volar plate, joint capsule, and extensor mechanism.

  • Evaluate a patient with a suspected finger joint dislocation by obtaining a detailed history, performing a focused physical examination, and selecting appropriate imaging studies to confirm the diagnosis and guide treatment.

  • Differentiate between types and directions of dislocations (dorsal, volar, lateral) based on clinical presentation and imaging findings.

  • Collaborate with interprofessional team members—including emergency clinicians, nurses, orthopedic surgeons, radiologists, and therapists—to deliver coordinated care.
