Activity Description

Prostaglandin analogs are synthetic compounds that mimic the effects of naturally occurring prostaglandins and are used for a wide range of clinical indications. They are used across multiple specialties, including ophthalmology, obstetrics, urology, gastroenterology, cardiology, and pulmonology. FDA-approved indications include PGF2α analogs (latanoprost, bimatoprost, travoprost, tafluprost, and latanoprostene bunod) for glaucoma; bimatoprost for eyelash hypotrichosis; PGE1 analogs (misoprostol and alprostadil) for NSAID-induced gastric ulcers, erectile dysfunction, and patent ductus arteriosus; PGF2α analog carboprost for postpartum hemorrhage and abortion induction; PGE2 analog dinoprostone for labor induction; and PGI2 analogs (epoprostenol, treprostinil, and iloprost) for pulmonary arterial hypertension, with iloprost also approved for severe frostbite. Misoprostol is also used off-label for labor induction, cervical ripening, pregnancy termination, and postpartum hemorrhage.

Prostaglandins are generally well tolerated, act locally, and require monitoring tailored to the indication. No specific antidote exists; supportive care is recommended in overdose. This activity provides a comprehensive review of the indications (both FDA-approved and off-label), mechanisms of action, routes of administration, adverse effects, contraindications, monitoring parameters, and toxicity profiles. This activity also emphasizes the interprofessional strategies to optimize the safe, effective, and evidence-based use of these medications.
