Activity Description

Uremic pericarditis is an inflammatory condition affecting the pericardium in patients with severe renal dysfunction, particularly those with end-stage renal disease or marked azotemia. The pericardium consists of visceral and parietal layers separated by a potential space containing a small volume of lubricating fluid. In uremic pericarditis, toxic metabolic byproducts accumulate in the bloodstream, provoking inflammation of this fibroelastic sac. Clinical features commonly include positional chest pain, a pericardial friction rub, and, in advanced cases, signs of cardiac tamponade. Uremic pericarditis is now rare due to improved hemodialysis practices, but its declining frequency may contribute to delayed diagnosis. Early echocardiographic evaluation is critical in suspected cases to identify pericardial effusion or hemodynamic compromise. While most patients improve with intensified dialysis, some require procedural intervention such as pericardiocentesis or pericardial window to relieve tamponade or obtain diagnostic tissue.

This educational activity enables clinicians to recognize, diagnose, and manage uremic pericarditis by strengthening clinical assessment skills and interpretation of key diagnostic tests, including electrocardiography and echocardiography. Participants learn to identify high-risk presentations, determine when urgent imaging is warranted, and initiate appropriate hemodialysis strategies or procedural interventions. Emphasis is placed on differentiating uremic pericarditis from other causes of pericardial effusion. Collaboration with an interprofessional team—including nephrologists, cardiologists, critical care specialists, and dialysis nursing staff—enhances patient safety, ensures timely diagnosis, and facilitates comprehensive care. By understanding the clinical nuances and coordinating across disciplines, clinicians improve outcomes and prevent life-threatening complications such as tamponade in patients with advanced kidney disease.

  • Evaluate the risks and benefits of various management strategies, including intensified dialysis, anti-inflammatory therapies, and pericardiocentesis.

  • Implement timely diagnostic protocols for uremic pericarditis that integrate clinical findings with echocardiographic and electrocardiographic assessments to guide decision-making in patients with end-stage renal disease.

  • Communicate interprofessional team strategies for improving care coordination and communication to advance care and outcomes in patients with uremic pericarditis.
