Activity Description

Diabetic embryopathy refers to a spectrum of congenital anomalies and complications resulting from maternal diabetes, particularly when hyperglycemia occurs during early pregnancy. Women with diabetes are 2 to 3 times more likely to have affected pregnancies compared to those without diabetes. The most commonly associated anomalies include neural tube defects, congenital heart defects, craniofacial malformations, limb deformities, and caudal regression syndrome—a condition uniquely linked to maternal diabetes. The risk of diabetic embryopathy is primarily attributed to the teratogenic effects of elevated maternal glucose levels during organogenesis, highlighting the critical importance of strict glycemic control through preconception and early pregnancy management in women with diabetes.

The risk of diabetic embryopathy is highest with poorly controlled pregestational diabetes, although early gestational diabetes can also contribute. Functional complications may include neurodevelopmental delays and long-term metabolic issues. Prevention requires preconception counseling and strict glycemic control (HbA1c <6.5%). Early detection through first-trimester screening, fetal imaging, and interprofessional care is essential. This condition highlights the vital importance of managing diabetes before and during pregnancy to reduce fetal risks and improve maternal and neonatal outcomes. This activity provides healthcare professionals with a comprehensive overview of diabetic embryopathy, encompassing its pathophysiology, risk factors, clinical manifestations, and evidence-based strategies for prevention and early intervention. This activity also emphasizes the importance of preconception counseling and maintaining optimal glycemic control during early pregnancy. In addition, this activity also highlights the importance of collaboration among interprofessional healthcare teams in delivering coordinated care for affected patients with diabetic embryopathy.

Target Audience

This activity has been designed to meet the educational needs of physicians, physician associates, nurses, pharmacists, and nurse practitioners.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Identify the teratogenic effects of maternal hyperglycemia on fetal development and the associated congenital anomalies.

  • Implement evidence-based strategies for preconception counseling and glycemic control to prevent diabetic embryopathy.

  • Apply current guidelines for prenatal monitoring, including first-trimester ultrasounds and fetal echocardiograms, in pregnancies complicated by diabetes.

  • Collaborate with the multidisciplinary healthcare team to develop personalized care plans that optimize maternal and neonatal outcomes in pregnancies complicated by diabetes.
